Pacientsky portál Community Wellness MyWellness je srdcom nášho programu Remote Patient Monitoring Plus (RPM+). Preberá a ukladá namerané hodnoty z vašich pripojených monitorovacích zariadení, aby vášmu poskytovateľovi primárnej starostlivosti (PCP) poskytol údaje na lepšie informovanie vášho plánu starostlivosti. Okrem toho môžete komunikovať so svojím klinickým trénerom a zúčastniť sa mesačných sedení v rámci aplikácie. The app first gave me grief. Uninstall, install etc. Then decided to read about how it works. Making change in what I did as to what I should have, what a difference. Haven't had to do anything but check my vital since. The program gives these wonderful people real time results and will contact me with in minutes depending on the results. It connects to the devices immediately, sometimes takes a minute or two. I appreciate how much it is helping me.

App could be a little more user friendly!! Since it says to keep the screen open, it is difficult when the cuff is working to try to keep that screen open!! Of course, it might be my phone too! I like having the results all in one place as well as the monthly check ins with Michelle, my wellness coach.

Heather was so helpful I am a phone illiterate person and that deer got me back on to my system and I'll be able to get all my numbers sent into you guys I've been taking them I just haven't been able to send them but yes she is great at what she does and I like her a lot and as for a rating I would give her a 10. This is a great and very helpful program... They help you with everyday life. They help you with medical supplies. The people is very caring. If they see you need something they're right on it they will call you no matter what time it is to check on you and make sure you're okay to give you medical advise and support...
