Use the Memorial Care app to schedule appointments, both in clinic or through our telehealth option, with our physicians and advanced practice providers, anytime, anywhere.
Urgent Care
You can use the app to locate the nearest Memorial Care clinic offering urgent care services and see how many people are waiting, as well as check in before leaving home using the “On My Way” feature.
If a clinic visit doesn’t fit your schedule, use the app to launch a virtual telehealth appointment. Using this convenient service, you can talk to an experienced Memorial Care provider via secure video link and get a diagnosis and treatment plan without leaving your home.
Patient Portal
Use FollowMyHealth or DMH Chart to access your health records, view test and lab results, request prescription renewals, talk to your doctor and more.
How It Works
The first step is to create a profile within the Memorial Care app. This profile will include your name, birthdate and basic contact info. Your profile also allows you to add dependents and create emergency contacts. Your dependents and emergency contacts can also save their spot in line using your Memorial profile. The app displays all your upcoming and past appointments.
NOTE: This app is intended for non-emergency use only. If you are in need of immediate medical assistance, please call 911.

The new self-scheduling part of the Memorial app is fantastic! I was able to schedule my son's physical with his doctor on the app. It even put it on my calendar for me. I did this on the weekend without ever having to call in. Best feature ever! The hold your spot feature for the Memorial Express Cares is also great!

The app won't let me put in my primary care physician, won't let me make changes, will only tell me how many people are in line at urgent care. I was hoping this would be a better alternative to their phone system but it's not

This is one of the best mobile app to check-in and also you can see your que number which saves a lot of time ...... just go for this app i highly recommend it.

I would give a 5 star if the app would work like it used to. I am not able to self schedule with Dr Hovland or any APNs at Koke Mill now and not sure if the app is broken or all of those providers are not using the self scheduling.

Best way to see the waitlist and book for the Expresscare based on the loctation. The UI is nice & simple for understanding.

Downloaded it to make a nurses visit appointment and that is not an option with this app. Why can i make appointments with physicians and PA's but not a simple shot appointment with a nurse?
