MESSAGE WITH YOUR PROVIDERS
Whether you have a question for your doctor, the nurse or any of the staff members you can easily send a secure message to the office using the app.
When you send a message it is sent in a closed and secure environment to the office and you will be notified instantly when a reply is available.
If your doctor is sending you a message, for instance with lab results this also appears on the app where you can read it right away.
No more waiting on the phone trying to get through to the office - you can now message securely with your doctor from your phone.
REMEMBER YOUR TO-DOs
It is one thing to have an overview of the tasks that one should complete to stay healthy - it is another to remember them all!
This is where your personal To-Do list comes into play. This tool shows you what tasks that you have to do as well as when and sends you a notification to remind you when it is time.
The To-Do list is based on the data in your Care Plan and combines all of your information, such as doctor appointments and medication refills, in one platform to make it convenient and easy to remember the important things for your health.
Whether it is to remind you to take your medication or exercise or monitor your vitals, an automatic notification will be sent to you ten minutes prior to the hour that the task is due

The test is quick and the results come with a recommendation on how to improve the aspects of your health which are not optimized as well as a praise on those which the requirements are met.
The results of your health test are also important for your providers, because it allows them to see which aspects of your lifestyle can be improved by improving your lifestyle habits.
It is recommended to take the test once a month, so that you will have an up to date set of data on how your health is progressing.
SHARE DATA WITH YOUR HEALTH CARD
If you are visiting a new doctor you can share your medical records by showing the doctor your Health Card on the app.
The Health Card can be used by any doctor worldwide and is a secure and easy way for you to bring your medical records with you wherever you go.
WORKS WITH YOUR DOCTOR'S SYSTEMS
MyCarePlan works seamlessly with your doctor's systems and will automatically be updated with data and recommendations from your doctor.
All recommendations from your doctors will automatically appear on the app giving you an overview of when to check in with your health provider, as well as the aspects of your health that you can monitor at home. This includes measuring your blood pressure as well as taking your medication.
When you add data to the app your doctor can access the data making it possible for him to optimize treatment and help you in between office visits

This app does NOT work at all. I even tried reinstalling it, no good. Will not even open, keeps saying it can't connect to the server. I completed all the forms and info on line on my computer and still the app does NOT work. I talked to tech, my model of Android phone is not supported so the app will not work, has nothing to do with the operating system, which is up to date and "should" work. Poorly designed app if it won't work for all Android phones, and if so, it should tell you that before downloading and going through endless misery. Finally, I was able use the service through the browser (Chrome) on my phone, which works fine. If you're having problems with the app, forget it and connect to My-Care-Plan through your browser (Chrome, Firefox, or Safari).

This MCP app is absolutely terrible. I strongly dislike how it requires connection via web browsers. As such, it adds confusion as well as having to alter browser security settings. Most recently, patients trying to connect to the waiting room are disconnected from their Carriers Data Network. The 5G/4G signals literally flash off and on, preventing connection. This has resulted in several missed appointments.

Horrible app, nothing works. I wish my medical company that makes me use it would discover how bad it is. They expect me to communicate with my therapist and them through it, but it has no working method for this. No recipients in the message part, no appointments listed although I have a standing appointment, nothing in news, etc. Worthless.

Awful app and terrible webpage. After fumbling on the webpage via desktop computer on firefox with no luck, I opted to get the excessively invasive mobile app. Selecting Health Card in the app loads the main dashboard but in landscape view. Starting the video conference directs off the app and onto a web browser. It's not indicated anywhere accessible that only certian browsers can accomodate. I finally directed the link to chrome and got myself set up. Nowhere visible to contact IT for help.

I can't see anything. I keep getting emails from my doctor saying I have information waiting for me in this app. When I open it, I'm already logged in. As I click through the menu options, one blank page after another opens up. Useless and frustrating.

The only thing that comes up are the titles and icons. There is a message, so an email told me, but i cant see anything. I have uninstalled and reinstalled and restarted my phone. I unfortunately dont know how to clear the cache on my phone. If there are anymore suggestions.

I couldn't connect with doctor an after reaching out to tech, they offer a fix but it didn't work. I lost my password an can receive a txt from app ( nothing I blocked that I know of) can only get password back via email ....but considering it doesn't work it's almost pointless

Easy install but confusing logon for first time.... finally walked away and tried again the next day. Couldn't write an e-mail. Instructed to click on icon to write a message but there was no functionality. Basically useless other than for reviewing profile info and check in.
