Email tells me to use app to complete health questionnaire but app asks for username and password which I have not received or been able to set up?! There are no other options to create an account or ask for help. Update: Turns out that you need to download the app from their link, not directly from the Play Store. Thanks for the response though!

Just get the NHIF format and transfer since that was the decision of the government, ndio ifike leaguey, other we are stressed enough, and were used to NHIF format. Email na pass word za nini sasa na you have the OTPs all over??? Ni kusumbua watu yet we are not getting results' easement for this idea.....
