WeRISE App是香港大學秀圃老年研究中心一個命名為「風起航：中風家庭支援行動」的計劃 (在2016-2018年由利希慎基金贊助) 的延伸。WeRISE App由嘉里集團贊助，並由香港大學中風研究組及香港大學秀圃老年研究中心合作開發，旨在借助科技為中風患者提供一個完善的風險因素管理平台。WeRISE App亦為家庭照顧者創建一個以「家庭團隊協助模式」運作的手機程式，以「檢測與監控、教育和支持」三管齊下的方式應對及管理中風。
